I, the participant or parent/legal guardian of the above-named participant, understand that participation in the Central Florida Dreamplex Social Skills Academy may include group instruction, role-playing, interactive activities, community-based practice, and other social and independent living skill-building activities. I understand that participation in these activities may involve certain inherent risks, including the possibility of physical injury, emotional distress, or other unforeseen circumstances.

By signing this form, I acknowledge these risks and voluntarily assume responsibility for the participant’s involvement in the program. I agree to follow all program policies, expectations, and procedures and, when applicable, to encourage the participant to follow reasonable instructions provided by Central Florida Dreamplex staff, instructors, volunteers, and program supervisors.

In consideration for the opportunity to participate in the Social Skills Academy, I agree, on behalf of myself, the participant, and our heirs, assigns, executors, and administrators, to release and hold harmless Central Florida Dreamplex, its employees, instructors, volunteers, representatives, and agents from claims or liability arising from injuries or damages resulting from participation in the program, to the extent permitted by law. I understand that Central Florida Dreamplex does not provide personal medical or accident insurance for participants.

In the event of an emergency requiring immediate action, and if I or the participant’s emergency contact cannot be reached, I authorize Central Florida Dreamplex staff or program supervisors to take reasonable steps necessary to protect the participant’s health and welfare, including contacting emergency medical services when appropriate.

I understand that the Social Skills Academy is an educational and skill-building program and is not a substitute for medical, psychological, therapeutic, or other professional healthcare services.
